Despite not getting much possession, Mainz almost broke the deadlock in an unusual way when the ball hit Lee Jae-Sung's head and hit the crossbar before Bayern regained control.
Bayern's first real chance came after 20 minutes, with Daniel Batz making a great save to deny Harry Kane from close range.
Batz was called into action again shortly after, making an impressive stop to tip Michael Olise's curling shot over the bar, keeping his clean sheet.
Mainz's defense was under huge pressure from the start, and it wasn't long before Vincent Kompany's team took the lead just before the half-hour mark.
Serge Gnabry pulled the ball back from the byline into the six-yard box, finding an unmarked Karl, who calmly scored, marking his third Bundesliga goal of the season.
Just before the break, Mainz stunned the home crowd with a stoppage-time equalizer from Kacper Potulski, who headed in a beautiful free-kick from William Boving.
Expectations were high for Bayern to dominate the second half, but they struggled, failing to register a shot on target in the first 15 minutes after the break.
Bayern fans thought their team was about to take the lead after 64 minutes when Gnabry had a one-on-one with the goalkeeper, but Batz made a great block before Kane's rebound was cleared away.
Mainz then took the lead midway through the second half when a long cross from Stefan Bell found Lee, who leapt to head the ball past Manuel Neuer, silencing the Allianz Arena.
Kane led the charge to equalize, but his close-range header was straight at Batz with just over 10 minutes left.
With time running out, the hosts were awarded a penalty after Potulski fouled Kane inside the box.
Kane stepped up to take the spot-kick, scoring neatly to beat Batz and earn a draw for Bayern.
